Classroom Documentation

Quality monitoring through data collections and analysis is an important aspect of the CPC approach. Information collected through the forms helps keep program implementation on track and helps school leaders and outside supporters understand the program benefits. These forms are ways in which program quality is measured and maintained.

Classroom Activity Report

Purpose: Classroom teachers individually and/or collaboratively reflect with their colleagues and the Head Teacher to determine the specific allocation of time in their classrooms and reporting the actual time spent on domains including: language arts/literacy, math, science, socio-emotional learning, art and music, physical education.

Instructions: Classroom teachers complete the report three times a year, noting the average percentage of instructional time spent in each domain. The Head Teacher or Curriculum Alignment Liaison (CAL) collects and enters them into an online portfolio (CPC recommends and uses ETO).

Classroom Learning Activities Checklist

Purpose: The CLAC records specific task-oriented behaviors and instructional supports that promote active participation and student engagement in preschool-3rd grade classrooms. A classroom-level observation tool is used. Scores from the CLAC provide a global measure of the classroom activities and interactions that directly facilitate task orientation and active engagement.

Instructions: Trained observers use this form to document the presence of active engagement and task orientation in preschool-3rd grade classrooms.

Curriculum Alignment Plan

Purpose: A plan of strategies aligns curriculum, assessments, and professional development within and across grades.The purpose of the Curriculum Alignment Plan is twofold: 1.) Compile preschool-3rd grade curricular activities, domains of learning, assessments, and learning activities into a seamless plan and 2.) Provide evidence of effectiveness, connections to assessment, communication across grades, and professional development planning.

Instructions: The HT and CAL, in collaboration with the principal, complete this plan with information each year in the fall, and it is signed by the principal. This plan is the foundation for curriculum alignment in the CPC program.

School-Home Agreement

Purpose: Parents read and sign the agreement as a sign of their commitment to participate in the program. The agreement is also signed by the principal or the Head Teacher to show the school’s commitment to provide a high-quality program.

​Instructions: Upon recruiting a child/family, the PRT or PIL assists the parent in completing and signing this document. A copy of this agreement is kept at the site.

Parent Involvement Log

Purpose: Schools use this to document the time each family participates in school events, volunteers at school, or attends the various programs offered by the PRT or PIL.

Instructions: Logs are completed monthly to provide the school with parent involvement data. This data is a way  to determine if families are fulfilling participation commitments.

Purpose: This is a plan of parent involvement activities for the school year and is based on the results of the Family Needs Assessment of each family.

​Instructions: The PRT or PIL completes this plan annually in mid-October with information for each of the years participating in CPC implementation. It is signed by the principal.
